Director of Admissions and Social Work

Serve as the central point for referrals, screening, assessments and placement for consumers. Represents CR in the community and with local and/or state officials. Serve as the point of contact for waiting lists.

ESSENTIAL FUNCTIONS FOR ADMISSION/ASSESSMENT:

• Completes all necessary intake and assessments, including record review, face to face interview, in home and behavioral assessment and consultation with service providers.
• Ensures completion of all nursing, medical, behavioral and other specialized assessments and integrates into assessment packet.
• Completes Integrated Summary and development of preliminary service plan.
• Acts as liaison between referring agent and Program Manager ensuring all admittance documentation is completed prior to admittance.
• Coordinates the Transitional Meetings in the programs with consumers, family and funders
• Coordinates the CR the Admissions Committee and participates in external admission meetings as necessary.
• Manages the waiting list for programs as necessary
• Acts as a point of contact and agency liaison for families, public and/or private agencies during admissions process.
• Compiles and prepares admission reports and outcomes that reflect the status of the admission process.

ESSENTIAL FUNCTIONS FOR SOCIAL WORK:

• Provides administrative and clinical supervision to the social work department
• Assigns, directs, and reviews the work of social workers providing services in the ICF/MR facilities and ensures that work is performed according to established ICF/MR policies and procedures.
• Serves as social work advisor to members of the interdisciplinary team as well as other professional staff, consumers, family members, and community agency representatives.
• Acts as a liaison with parent associations and other community-based service providers; and identifies and develops available social service resources in the community for consumers
• Compiles and prepares clinical reports and outcomes related to the activities and accomplishments of a social work program.

OTHER JOB FUNCTIONS

• Works with stakeholders in respective communities to develop necessary referral sources for CR programs
• Participates in identifying consumers in need of service to include acting as a representative at attending provider fairs
